Requisitos

·      Fotocopia de la partida de nacimiento del menor

·      Fotografía a color tamaño pasaporte, fondo blanco. En éste caso, personal del RENIEC tomará la fotografía GRATUITAMENTE  en el local de la Orden Profesional y en las fechas y horario indicado ( 9 y 10 de feb.)
